  • StatusWell is a healthcare platform that offers personalized diabetes and weight care to individuals. Our providers utilize continuous glucose monitoring technology paired with an AI-enabled app to customize a treatment plan that fits your individual healthcare goals and needs.
  • Select Get Started to complete our initial questionnaire and provide your contact information. Once submitted, one of our specialized wellness coaches will reach out to you for further details and to schedule your initial appointment with your provider.
  • Our programs focus on diabetes care, diabetes prevention, and weight care. Depending on your medical history and specific health needs, our care coordinators and providers will match you with the specific program that works best for you.

  • If eligible, StatusWell’s programs can be fully or partially covered by your insurance. If your insurance plan does not cover our services, we offer affordable subscription plan pricing that fits your needs.
  • We are currently available in Connecticut, Florida, Maine, Massachusetts, New Hampshire, Rhode Island and Vermont.

Programs

  • All of our programs include access to our StatusWell app which allows you to meet with your provider via telehealth to customize your treatment plan and goals.
  • Our providers initially prescribe continuous glucose monitors (CGM) to gain better insight on your current health status and your personal needs.
  • Your provider may prescribe necessary prescription medications to help you achieve your goals. You’ll also get access to our partnering pharmacy medication services.
  • We also offer personalized wellness and diabetic education coaching, live diet monitoring advice and reinforcement via Chat, and optional customized diet plans.
  • Continuous glucose monitors (CGM) are devices that track your glucose levels (blood sugar levels) in near real-time. CGM requires the application of the CGM sensor generally to the back of your upper arm where the sensor can constantly read your glucose level for 14 days. The CGM sensor sends this information directly to the StatusWell app where you can view your glucose trends over time. Your provider will use this information to adjust and personalize your treatment plan.
  • Our providers can prescribe FDA-approved medications for diabetes and weight management. We don’t prescribe medications that are classified as controlled substances. Your provider will prescribe medications based on your past medical history, current needs, any allergies you have, and your current medications. In some cases, our providers may prescribe GLP-1 medications. This is a class of medications that have been FDA-approved for diabetes and weight management.

  • StatusWell currently serves adult patients aged 18 or older.

  • Yes, we’ll work with your PCP to make sure they are in the loop on your care.
